Saying CoD runs on Quake is like saying all of Valve's games run on Quake.
Both the IW-engine and the Source (and GoldSource) engine use Quake as a base, but it's heavily modified to the point that it isn't even recognizable. No-one even bothers even mentioning that because they're practically their own engines now.
Not sure how you can say that it wasn't designed for the purpose. An engine can usually be modified and changed and starting from scra...
... but it isn't the same game. (Arguably, it's much worse if you want to be negative about it.) It has had changes with each iteration.
If CoD hasn't changed since CoD4, then by that logic, most sequels are also in the same pit.
What stress is the engine being put through? It's a small number of players in small maps. Nothing really stressful going on there.
A new engine would end up changing the shooting mechanics that so many love about CoD....
I remember Half-Life's expansions got criticized for being only 6-hour long... which was deemed short even for an expansion.

The only issue so far is that not everyone is hooked to the net. That can change within half a decade.
Movies are almost on the point of being digitally distributed as well as games on the PC... console games will follow shortly down the same road.
