I'm doing a search for "plastic surgery GTA online" and am finding literally nothing. So I'm not sure what amazing option you are talking about. And even if they have plastic surgery, it won't change the basic ugliness of the online character models or the lack of variable body shape. Nor will it compete with SR's system.
That it's unique and has never been tried before doesn't mean anything. Here are some reasons it sucks:
1. It is massively limited.
2. The characters look ugly.
3. You can't change the character's body.
4. While GTA is great, one of its competitors - Saint's Row - has one of the best character creation systems out there. I'm sure this won't encourage anyone to leave GTA for SR, but it shows the potential in this market, a po...

You'll probably lose your character. It seems that if you create a character and can't host, you lose the character save. Not sure if it's universal, but it's happened to me every time.
It sucks. Massively disappointing. There's no reason they couldn't have included some of the cooler features, like lifestyles choices, within something like a Saint's Row-esque creation system.
From my experience with it earlier yes, you have to choose them.

I'm about 60% through the game and have not played online yet. But I'm open to the view that it's overrated. I can see why people would say that, but I'll reserve judgment for now. Still great, but not quite what it might have been.
