Yup, while games being labeled as mediocre like Black Ops 3 will have 4 player co-op online or split screen, likely running 1080/60... At least on PS4. Not to mention not separating single player progress from co-op progress.

You will, this story actually confirms it. To start 2K16 they used 2K15. Same would be said for '14 and if, big if, THQ told the truth about '13 the engine was reworked but not a completely new engine.
You can also find evidence of copy/paste happening if you start backtracking through the games and finding the same existing glitches, glitches that were present for some time and never fixed.

Yup, pretty much. So the campaign can be played in Co-op or single player without losing progress, even if you jump in with a friend ahead/behind you.
I play both. I like to play the campaign first as then I can come to grips with the new guns and play style.

Microsoft actually stated this before Windows 10 for PCs came out. Mid-year for PC, by the end of the year for other Microsoft devices.
