For ppl having network issues, are you playing after your PS4 was in standby mode?
I noticed mine did this. If you restart your machine and play, you stay online and match with host or guest 100%.
The game has within its code an executive to drop blood vials when you're low.

I dont think many of the commenters have any idea of what the point of grinding and farming is. Too much Mario or CoD? What the hell causes this?
RPGs and grinding go hand in hand. Its part and parcel of building your guy, and it is not an exploit. It is the dirty work that's done between missions.
Grinding defeats the whole point of playing an RPG?
You must lose every single PVP encounter from your ivory tower, and that is not the point of the game.

In the other games, you had to pull out a piece of moss and gently insert it into your mouth and slowly chew, or remove the Esty cap and gulp the booze.
This game is different, but the same. Its hard, but more forgiving.

I have 40 insight now, from just consuming a whole bunch, and enemies got way tougher.

I thought it was just that enemies get different attacks or weapons based on your progress, not insight.
Same trigger as what causes NPCs to move, or the appearance of those dead guys who first bring you to Hordrid Gaol or Unseen Village.

This game is nothing even remotely resembling hack N slash. You may use an edged weapon, but the similarity ends there.

I think throughout my life, I have sent 2 or 3 Atari joysticks to their grave, numerous NES pads, Super Ninty pad, a few PS1 and 2 sticks and one Sixaxis. (not all from rage breakage)
The DS4 lived tho! It's built better. You can chuck it, just don't spill coffee on it. SONY hasn't developed a coffee proof stick yet ;) My launch DS4 did not survive the Java incident :( call the CAS on me. I am guilty!

Nothing I have discovered thus far has reached the level of difficulty of defeating the 3 Ruin Sentinel in DS2. That boss was probably the most challenging of them all. Maybe because it was so early in the game and there was no other choice but to fight it.
The game should be played by everyone. I have used the beckon bell a couple times and it works great!
Exclusives drive machine sales. Its not stupid at all.
The system is entrenched in gaming because it is lucrative for the publisher/console maker. Its part of building a brand; what separates your machine from the competition.
I think all systems having the same game would be stupid.
Craziest thing just happened to me!
I was fighting Rom and all out of vials and I had even used my special blood items.
He was down to about 20% and I was about 50% and right at that moment someone out there rated a note "fine" and my health went all the way up and I won!
Talk about epic. That was the coolest moment for all my Souls battles. That was amazing as hell!
