Yes - any enhancements to PS4 games need to be done from the internal SSD.
All games can be stored externally, but again - PS5 games will need to be on the internal SSD to play.

I think it will be difference with most 3rd party games. Going off people's glowing praise of it - seems any other method will feel like it's missing something.
