Yah, the codes are most likely one time uses. Just see if when your friend goes to GameStop to pick up his copy when the game comes out, that he asks for another code.

Online create isn't that big of a deal. I'd rather create my own levels by myself than have a friend joking around and killing my designs and making it take longer to make. Also, I'd rather surprise my friends with the levels I create than them knowing it inside and out. I don't think most people will care too much about it missing, as the fun will come from playing levels online with their friends.
I don't think all of that was user created levels. I remember seeing the cow rodeo level in a video from Leipzig I believe, not sure of the others.
