Apparently sword impaling is ok in Japan :P It's the west that has to censor it. An example is the recent tales of berseria. The japan release had a similar scene which was changed for the west release
There is a single player story, it's just not in the beta! Personally i don't expect too much from the story though, its main focus is the multiplayer it seems :)

If i recall correctly, there was something similar done only in japan for the release of twilight princess. The gamecube version of the game was only available there from nintendo itself. I guess it was done to promote the wii over there.
For the rest of the world it was handled normally as one would expect. Perhaps this could be something similar?

if you can, try to play tales of vesperia. My favorite by far :) Only on 360 though. Hopefully it will get backwards compatibility with xbone at some point.
But all the games are awesome. The 2 xillia games on ps3, and graces as well :)

the smoke you see I would guess is the standard "fog gate" that exists before bosses in all souls games
There was a ps3 version of the game that came later with all this new content. But it never came west, it remained in Japan only
