1050/1050 on achievements for me, and I found Insane mode to only be slightly harder than Normal, especially with the soldier class. Playing as a Biotic or Adept had its difficult moments, but nothing insurmountable.
What they really need to work on is the framerate. As much as I love this game (easily one of my favorites), the framerate just labors at times.
Who died depended on who you left behind. There were two choices, plus a third if you couldn't convince a certain character to continue to follow your lead (and had to kill him).
Bioware has insinuated that you might be able to use your Mass Effect gamesave to affect Mass Effect 2...so if you left a certain character to die in Virmire, that person would not be available in ME 2 (again, if you chose to use your old save). Also, your moral path, be it Paragon or Renegade, may also ...

I still play it (campaign and online) about once a week, and am very much looking forward to the expansion.
I don't think it quite lived up to the hype, but I'd still give it a 9.5 out of 10. (The expansion, depending on quality, may raise or lower that number).
A few requests for the sequel:
--More playable species. Imagine being a Krogan, for example
--Greater weapon variety
--Better framerate, please
--Less time in the Mako. Driving that thing sometimes proved to be an exercise in frustration
--More biotic powers
--Better variety in the sidequests
--Make the Paragon/Renegade choices have real impact on the storyline as it progresses
