Hidden (characters and/or bonuses) turned into DLC. Companies probably figure, hey why should we give bonuses when we can make these suckers buy them. It's a very unfortunate reality.

shots look in-engine, not necessarily in game. Saw something similar, way back when Halo 2 launched. High res shots taken from a PC and not necessarily an Xbox. I never trust bungie because of it.
