.02% failure rate is nothing to be concerned about. A lot of these stories are surfacing from the same one or two sources, while others are baseless without evidence.
Xbox 360's failure rate was 50%+

A lot of the people making these stories up are just desperate that the PS4 would at least have some issues. They want balance. But the truth is that every issue the PS4 has had has been a nonissue. .02% failure rate is honestly so low, you'd have a really hard time finding a dead PS4 of your own. The rest, like lack of MP3 at launch (patch already confirmed) or server overload crashes are normal. Wtf what do we expect at launch, perfection? No one can predict how crowded your servers wil...

Parity starts early in development, and they know before the game is released that it's going to be the same on both consoles. From early stages, they begin gimping one version to meet the other's "full potential", which is why Microsoft is frowned upon with their ridiculous, but...
By design, all multiplatform games should look better on PS4, as it is the most powerful console ever made and easiest to develop for. If it look worse or on par, we'll know they did it on purpose.
