Fallout 3 looked great. I don't know what you're talking about.
Not in open world games. For PC sure, but not on console. GTA 5 is 30fps, Watch Dogs 30fps, Destiny 30fps, Witcher 30fps and so on.
It is in game, but keep in mind that the screenshot only focus on one small house a few meters away. If you watch the trailer you will see that the stuff around the house is not all rendered with the same detail. Draw distance come to mind.

Oblivion was that "out of this world-ish" when it arrived. Graphics was a major talking point. The same was Morrowind. Not quite as much in Fallout 3 and Skyrim.
