It was Metal Gear Solid Portable Ops that had the user activated Fulton system before MGS:PW. To my knowledge you couldnt use it to extract other soldiers, it only allowed players to leave the mission area without receiving further injury.

My experience with GZ I noticed that the enemies would do two things to notify HQ of your presence. If they hear or noise or see something suspicious from afar, such as a man in a sneaking crawling, or you walking and them being unsure about your identity, they'll tell HQ about this noise or suspicious activity over the radio, once HQ clears them, you can shoot the radio (I tranqed), but that doesnt stop them from yelling out "Intruder".

They had to close the series, and answer questions.
While the AO capsules were successful in grounding dynamic objects and characters in the scene, they had their own limitations, particularly on a highly-reflective material. On such a material, these capsules would appear ridiculous and show cruder silhouette of the characters. The devs called this effect the “Lycan Child” and to avoid it, they disabled it on these highly reflective materials.
