You mentioned ps4 titles that are avaible on PC, what about the ones that aren't? Games like Uncharted, God Of War or GT will never be on PC.
There is a big difference,Im sure this will increase the sales figures overall, but people who use PC will have even less of a reason to buy a Xbox One.

There are mods for skyrim wich change the combat completly,I remeber there being one mode wich added a wholne new story and quest and it was better than most DLC's we get.
I don't agree with charging for mods, I believe people should donate if they want to, but some of the mods ARE worth it.
There is plenty of info on the Hunter's Dream and I believe the game as a manual.
If actually want to understand the story you should talk to every NCP you come across and read items descriptions,if you still don't figure it out by the end of the game you can read about it online.

If you're skill stat is low of course you'll get less from that weapon, if you're going for a strength build I would recommend the kirkhammer,Ludwigs blade or the wheel.
If you kill the enemy that killed you previously you'll get your blood echoes back ;)
I recommend trying it, if you have a friend that you can borrow it from it would be best.
Anyway, the gameplay is a lot faster than demon souls and it revolves a lot more about dodging, so I think you should give it a try. ( I am a bit biased because im so hyped for this game!)

Lots of people asked for TLOU,GTA V and some other games to be on new consoles/PC, I think thats why those games are being remasterd
It's a new generation, there arent that many AAA titles to give out on PS plus, but im sure they will later just like they did with the PS3

Sorry but both have had more than "average" exclusives. On the ps4 you have Bloodborne which is a masterpiece, on xbox you have forza wich was also a great game, just because you don't enjoy a game doesn't make it any less good.
