Ether_Snake from NeoGAF made it, then GT started using it... it kinda picked up from there. He go the idea from the beard-shaving and the battle damaged suit ideas that had been talked about in interviews.

Great, we finally get co-op but it is online... I hope they have a splitscreen set up as well...
It's a good thing you have never played a MGS, it would "break you" ;)... The games have a lot of cutscenes, but almost twice as much gameplay...

Yeah, it had some scenes that we have seen before, but they have been updated. The character models are different than they were originally in the trailers and the color scheme is quite different...
I believe Ryan said that they only eliminated the ability to choose frequencies... The optional codec scenes are still available, but it is all set up like the memory function of the original three. You can still call anytime, but you will be choosing a character's name/specific frequency.
A phrase more commonly associated with movies.... I think it may have something to do with how long they watched it... From my understanding, a lot of rating is done by watching select parts of gameplay along with the cutscenes... Maybe that was the rating time...

Seems like they have added a ton (Mark Mac voice) of new features.
The only thing I am weary about is the over-the-shoulder perspective... I don't like that in my fighting games, but I may have misunderstood what they were saying...
