The survey says: "We will be recruiting for a variety of FIFA experience, from non-FIFA players to PES fans to hardcore FIFA players. Please answer the below questions honestly - those selected for participation will be contacted on via email soon!"
Playtest sessions look set to take place between Tuesday April 30 and Friday May 3.
EA officially announced FIFA 14 for Xbox 360, PS3 and PC last week, noting that "additional formats" will be revealed in the coming months.
